Cost is $55.00 per child

Download Forms and Mail In
We suggest using Option 1 above, but you may also
print the registration form,
fill it in by hand,
and mail it in.
If you are printing the registration form,
please print this page and follow
these instructions:
- Print one copy of the registration form for each child being registered (ages 4-16
only please).
- Complete and sign one form for each child. Please leave the MGSA USE ONLY block
empty.
- Make three copies of each form (four including the original). This is important,
entries received without the full four copies of each form will not be processed.
- Payment Methods:
- Payment Method 1 - Enclose a check or money order payable to Midlothian
Girls Softball Association to cover the registration fee of $55.00
per child being registered.
- Payment Method 2 - Make a payment to payments@midlothiangsa.com
using
PayPal, which will allow you to use a major
credit card to make your payment if you wish. Make the payment in the amount of $55.00
per child being registered. Print your PayPal receipt and attach it to your mailed
in registration forms.

Important: Any currently available discounts are considered in the above costs. If payment is not
received before any discount deadlines, the amount will not be enough to cover the
